The Role
You'll provide skilled nursing care to residents in assisted living and long-term care facilities across the North Canton area, including Hudson, Tallmadge, and Cuyahoga Falls. You'll assess patient conditions, administer medications, coordinate with facility staff and physicians, and maintain clinical documentation. This role offers meaningful independence while keeping you connected to a professional care team.
What You'll Do
- Deliver skilled nursing care and perform assessments in facility settings.
- Monitor vital signs, symptoms, and patient status; report changes to physicians and care teams.
- Administer medications and treatments as ordered; document all clinical care.
- Collaborate with facility staff, administrators, and physicians on patient care plans.
- Build strong, professional relationships with residents and facility teams.
- Manage your own schedule and workload within your assigned territory.
- Maintain high clinical and documentation standards.
What You'll Need
- Current Registered Nurse (RN) license in Ohio.
- Experience in assisted living, long-term care, skilled nursing, or hospital settings.
- Strong communication skills and professional accountability.
- Comfort working independently with self-direction.
- Commitment to quality patient care and clinical standards.
- Reliable transportation and willingness to travel within assigned territory.
Nice-to-Haves
- Prior facility-based nursing experience.
- Background in care coordination or case management.
- Experience with point-based or productivity-focused compensation models.
What We Offer
- Point-based pay: $48–$52 per point based on service complexity and your experience.
- Performance bonus: 10% additional pay on points earned above 30 per week.
