What You'll Do
- Evaluate patients and develop individualized treatment plans in their homes.
- Provide skilled physical therapy interventions and monitor patient progress.
- Communicate with patients, families, and physicians about care plans.
- Manage your own daily schedule and caseload with minimal travel time.
- Complete accurate clinical documentation in a timely manner.
- Build strong, meaningful relationships with patients and care team members.
- Maintain high clinical and professional standards in all patient interactions.
What You'll Need
- Active state Physical Therapy License (PT, DPT, or equivalent).
- Experience providing direct patient care in any clinical setting.
- Strong communication and organizational skills.
- Ability to work independently and manage your own schedule.
- Reliable transportation and valid driver's license.
- Professional approach to patient care and documentation.
Nice-to-Haves
- Home health or home-based care experience.
- Experience with electronic health records (EHR) systems.
- Current CPR certification.
- Interest in building long-term patient relationships.

How Compensation Works
Instead of hourly pay, each visit earns points based on time, complexity, and clinical responsibility. You control your earning potential—take more visits for higher income, or maintain a balanced caseload. Specific point values are discussed during interviews.
