ROLE SUMMARY

As a Health Coach, you'll be the trusted point of contact for patients working toward significant health and lifestyle goals. You'll conduct regular virtual check-ins, track progress, coordinate care with our clinical team, and provide the motivation and support that helps patients succeed in managing chronic conditions and building healthier habits.

KEY RESPONSIBILITIES

• You'll conduct weekly or bi-weekly virtual check-ins with patients using telehealth platforms to assess progress, celebrate wins, and troubleshoot barriers to success.

• You'll support patients in setting and achieving nutrition, activity, and behavior change goals tailored to their individual needs and readiness for change.

• You'll serve as the primary point of contact for patient questions, concerns, and support requests, responding promptly and with empathy across digital communication channels.

• You'll coordinate care by communicating regularly with physicians, registered dietitians, and other care team members to ensure patients receive integrated, holistic support.

• You'll document all patient interactions accurately and timely in our electronic health record system to maintain continuity of care and proper clinical records.

• You'll identify and escalate clinical concerns or red flags to licensed providers immediately to ensure patient safety and appropriate clinical oversight.

• You'll develop and refine your coaching techniques by staying current on best practices in health behavior change, obesity medicine, and chronic condition management.

REQUIRED QUALIFICATIONS

• 1-5 years of professional experience in health coaching, patient navigation, care coordination, or a similar patient-facing role.

• Proficiency with electronic health record systems, telehealth platforms, and digital communication tools; ability to learn new software quickly.

• Strong organizational skills and the ability to manage multiple patients and concurrent touchpoints without losing track of details.

• Demonstrated ability to build trust, communicate with empathy, and motivate patients in a virtual setting.

• A reliable high-speed internet connection and a private, quiet workspace that supports HIPAA compliance.

PREFERRED QUALIFICATIONS

• Active health coaching certification from a recognized program or a bachelor's degree in nursing, health sciences, nutrition, or a related health field.

• Experience supporting patients with obesity, weight management, or lifestyle medicine.

• Familiarity with chronic disease management, behavior change models, or motivational interviewing techniques.
