Occupational Therapist (OT) - Home Health

Location: Cleveland, OH (Cuyahoga County)

Compensation: $60-$65 per visit

Schedule: Full-Time

PTO: 4 Weeks

What You'll Do

  • Evaluate patients and develop individualized care plans based on clinical assessment and patient goals.
  • Provide skilled occupational therapy in patients' homes focusing on independence, safety, and quality of life.
  • Train patients and caregivers on adaptive equipment, home modifications, and functional techniques.
  • Document visits using electronic medical records (EMR) and point-of-care systems.
  • Manage your own caseload and daily schedule with full autonomy and support.
  • Communicate regularly with interdisciplinary care teams, families, and patients.

What You'll Need

  • Current Occupational Therapist License (required).
  • Reliable transportation and willingness to travel within Cuyahoga County territory.
  • Comfort working within a point-based compensation model.
  • Strong communication skills with patients, families, and clinical teams.
  • Commitment to professionalism, patient-centered care, and clinical excellence.
  • Ability to work independently while collaborating with team support.

Nice-to-Haves

  • 1+ years home health or outpatient therapy experience.
  • Experience with Activities of Daily Living (ADL) training, home safety evaluations, or adaptive equipment.
  • Familiarity with electronic medical records (EMR) systems.
  • Experience with functional assessments and care planning.
