Summary

Location for this position is at the CODY, WY, Community-Based Oupatient Clinic (CBOC). Responsible for providing in-home rehabilitation therapy assessments, home assessments, treatment planning, & management of rehabilitation in the home for patients in the HBPC Program. Provides in-home services to veterans who present with a wide variety of multiple, complex chronic illnesses &/or profound disabilities to improve or maintain functional levels & encourage independence in the home.

Duties

  • Develops and carries out the Physical Therapy Treatment Plan to meet individual rehabilitation objectives through interventions such asl therapeutic exercise, muscle testing and muscle re-education, ROM, cognitive therapy, neuromuscular facilitation, perceptual-motor integration, fabrication, and application of devices for daily living, teaching adaptive or compensatory, techniques for accomplishing tasks, providing patient/family education regarding disability. Responsible for independently providing assessment and treatment interventions to inpatients and outpatients received through healthcare provider referrals and patient self-referrals. Interpret findings from examination, diagnostic studies/medical tests and, during evaluation, select appropriate clinical tests and measures, and then integrate findings into the physical therapy plan of care for the full range of patient populations. Establish a diagnosis within the scope of physical therapy and identify the appropriate intervention to treat and/or refer to another provider for further consultation when needed. Reevaluates performance with appropriate modifications to treatment as indicated. Presents patient to physician with information regarding outcomes of treatment and status and expectations. Communicates effectively both orally and in writing, adapts assessment tools and treatment to the complexity of the diagnosis or disabilities, and demonstrates the clinical reasoning necessary to identify the need for non-standard assessment, methods and techniques. Requirements

You must be a U.S. Citizen to apply for this job.
