Summary

The Community Care (CC) Coordinator Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. This RN uses nursing knowledge and systems theory to assess, organize facilitate, and guide Veterans through their full range of care options including internal/direct care in VA and care in the community. *** This is a Temporary Not to exceed 9/30/2029 Appointment with an Intermittent work schedule

Duties

  • This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until Sept 30, 2026. The initial cut-off date for referral of eligible applications will be July 27, 2026. Eligible applications received after that date will be referred at regular intervals on an as-needed basis until positions are filled. All referred applicants receive a final notification once a selection decision has been made. Major duties include but are not limited to: Coordinate timely, Veteran-centered care for Veterans receiving services through VA and community care pathways.- Triage requests for services and help direct Veterans to appropriate care options based on clinical needs and established processes. Develop and support care coordination plans that promote continuity, access, and successful completion of the episode of care. Review medical documentation from community providers to support informed care coordination and follow-up. Serve as a liaison between Veterans, VA care teams, and community providers throughout the consult and referral process. Guide Veterans through available care options, including direct VA care and care in the community. Collaborate with interdisciplinary teams to support timely referrals, scheduling, communication, and care outcomes. Document, exchange, and track care coordination information using VA health information systems and required care coordination processes. Pay: Competitive salary Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: This is an intermittent position. Intermittent employment is defined as non-fulltime employment in which employees are appointed without a regularly scheduled tour of duty. Work is sporadic and unpredictable so that a tour of duty may not be regularly scheduled in advance. Telework: Not Authorized Virtual: This is not a virtual position. Requirements

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with VA. Selective Service Registration is required for males born after 12/31/1959. Subject to background/security investigation.
