The incumbent functions as a Licensed Professional Mental Health Counselor (LPMHC) in the Mental Health Residential Rehabilitation Treatment Program (MH RRTP) located at the Dayton VA Medical Center.
Duties
- The incumbent functions as a Transitional Care Manager whose primary responsibilities are to provide a full range of direct patient care services, including psychotherapy and discharge planning, to MH RRTP Veterans diagnosed with a range of Substance Use Disorders, Mental Health Disorders, and experiencing psychosocial issues including homelessness. Duties: Provides a range of direct patient care services, to include individual and group psychotherapy, psychoeducation, assessment and diagnosis, psychoeducation, family/couples therapy, and case management/discharge planning. Engages in psychosocial and diagnostic assessments, establishes therapeutic rapport, administers therapeutic interventions, and ensures continuity of patient care. Establishes a proactive, resourceful approach to discharge planning. This includes initiating, coordination, and expediting the successful discharge of Veteran clients. Participates and contributes to a multidisciplinary treatment team to provide comprehensive services. Effectively communicates with and utilizes VA network and community agencies to facilitate treatment, discharge planning, and continuity of care, this includes engaging in referrals to VA and community agencies and government programs as needed. Demonstrates regular contact and collaboration with a variety of VA and community agencies and resources in a manner that enhances coordination of comprehensive patient care, and/or problem resolution. Participates effectively in team meetings, unit huddles, treatment planning conferences, and other teams and meetings as assigned. Documents in EHR in accordance with VHA Directive and policy. Physical Requirements: The duties include sedentary and ambulation (walking) throughout the medical center and in the community according to the demands of daily tasks. Light carrying, less than 15 pounds-may be required as part of duties related to storing or moving files. Working around moving objects or vehicles is required as part of daily ambulating within the medical center around patients who use motorized scooters and wheelchairs. Working along duties requires working along at intervals during the day (i.e., documenting progress notes, completing phone calls, etc.). Reaching above the shoulder- may be required during daily tasks to access files located in overhead cabinets. Walking- required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other medical center departments as needed to complete duties of the position. Standing required approximately 3-4 hours daily associated with walking from assigned office to outpatient clinics and other departments within the medical center as needed to complete duties of the position and filing patient records as needed.It is required that this individual be able to operate a motor vehicle as this position and it is expected that the incumbent transport patients off-site as part of their duties. Work will take place in- and outdoors thus exposing them to the natural elements of the weather.The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. May be exposed to infected patients and/or contaminated materials and will be required to take appropriate safety and protective measures.
