Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. In addition, the incumbent provides geriatric and/or palliative coordination and oversight for the CLC. The following duties are required of the position; they include, but are not limited it: Coordinates comprehensive, interdisciplinary, medical care and case management in CLC for Veterans with complex medical, social and behavioral conditions. Participates in the establishment of policies, procedures, and guidelines necessary for clinical care in collaboration with other members of the interdisciplinary team. Acts as a collaborating physician for APP providers in the CLC. Monitors performance of medical care provided by Physicians and providers in the CLC. Oversees admissions to the program. Provides consultation to the medical staff for patients with complex treatment or care needs in the home. Collaborates in the monitoring of quality and safety concerns within the program. Communicates relevant responsibilities, policies and procedures to providers. Responsible for maintaining medical standards of care, as mandated by accrediting bodies such as the Joint Commission. Work Schedule: Monday - Friday, 8:00 am to 4:30 pm
